Social Services
Our Chaplain is a Master’s level licensed minister and certified chaplain who offers spiritual support to our residents and family members. She provides prayer for residents individually in addition to facilitating Ecumenical, Catholic, Rabbinical services, and any other spiritual services our residents may require. She leads the inspiring and moving “Celebration of Life” memorial services, and has been honored to perform many private services.

With a combined 36 years of experience, our Social Workers
play an integral part in the assessment, initial welcome and continuing care of
our residents. Upon arrival, one of our social workers will visit the new
resident and get to know them through social history, cognitive and mood
assessments. We will arrange a family conference during which the resident and
their loved ones may partner with our staff in determining a plan of care. The
highly credentialed social workers at Meadowbrook are a wealth of knowledge
regarding Medicare/Medicaid and community resources, including facilitating
voter registration, psychological services, and our innovative “Celebrate Me
Home” discharge planning services.
