Please use our Nursing Home Checklist for help with narrowing your nursing homes choices. The checklist provides questions and observations that are important to keep in mind as you visit nursing homes and will help you make a good choice for you or your family member .

• Meadowbrook accepts admissions 24 hours a day, 7 days a week.

• We recommend a personal tour of the facility to familiarize yourself with the environment and to meet our staff.

• State and Federal regulations on Long Term Care Facilities (LTCF's) require a history and physical (less than 30 days old and signed by a physician), a list of prescribed medications and treatments (signed by a physician), a T.B. test and a pre-admission mental illness screening. Medicaid applicants are required to have a completed "level of care" authorization from the Department of Human Services in their county of residence.

• Admissions are accepted based on the chronological order in which inquiries are received as well as the appropriateness of the available unit and room.

• Each resident is entitled to their physician of choice and Meadowbrook welcomes any physician who chooses to follow his or her patient to our facility. The physician must see the new resident within 48 hours of admission and monthly thereafter. In the event you need a physician Meadowbrook has an approved list of attending physicians from which to choose.
